Booking information 

To Book your child's place at an All 4 Sports club, please complete all parts of the form to the right
 Should the club be fully booked, we will contact you immediately to let you know. If you do not hear from us then your child has been successfully enrolled
 On the day of the club, please make sure that your child brings the appropriate footwear and clothing. if your child is attending a football club then shin pads will be required, for safety reasons
 At the end of the school day, your child will be met from their class, or at the designated meeting point. They are then registered and taken out for their session. At the end of their session, each child will be de-registered and taken to meet their parent or quardian. if your child makes their own way home, you must let us know in writing
 In severe weather conditions, please note that sessions will take place indoors.
